2023 EP Update Express Enrollment Form

What is included:

• Current updates to ONE foundation trust and other legal documents we have created for you to include the 2022-2023 statutory and tax law changes; name changes for beneficiaries and changes to the office of trustee, health care representative or personal representative.
• Current updates to your ancillary documents including your health care documents for you, and your durable financial powers and last wills.
• Telephone consultations with members of our staff on matters relating to your estate and DocuBank health history.
• Assistance with funding accounts, assets or properties acquired after your initial funding into your trust.  However, the draft of any special document, such as a Special Warranty Deed, will be billed at our normal rate.
• Annual review of assets to make certain both the Plan A assets and the Plan B assets are under control during your lifetime as well as after your death.
• Our team will interact upon request with your other professionals such as your CPA or financial advisor; or with family members as you direct.

What is not included:

• Estate Plans for individuals who are not current clients, or not offered this benefit.
• Two trust families will pay an additional per person fee.
• 4 Meetings are included; additional meetings will be billed at our normal rate of $450 per hour.
• Upgrades or additions to your plan that involve new circumstances such as death of spouse, marriage or divorce, different trusts, and separate business entities.
• Costs incurred such as FEDEX, express mail or priority mail charges, filing and recording fees, and on certain occasions, photocopies and postage.
• Other attorney time which includes matters unrelated to your estate plan.

I have read the forgoing and wish to enroll in the 2023 EP Update Express Program ^TM

Enrollment fees are as follows:

$700 per person if paid by Monday, October 31, 2022*

$800 per person if paid between November 1 and December 1, 2022.*

After December 1, 2022 the program will no longer be available until 2024.

*Two trust families will pay an additional per person fee.

All Appointments must be held during the 2023 calendar year.

I understand that, as a result of the reduced amount of the fee, as well as the level of work to be provided by Ilene L. McCauley, Ltd. the fee will be earned upon receipt.
