2026 EP Update Express Enrollment Form

What is included for preapproved clients:

• Updates to ONE foundation trust and related legal documents we created for you, including new statutory/tax law changes, beneficiary updates, and changes to trustee, health care representative, or personal representative.
• Updates to a TWO trust plan will cost an additional $500.
• Updates to your ancillary documents: health care directives, durable financial powers, and last wills.
• Telephone calls and emails with our staff regarding your DocuBank health history.
• Assistance with funding accounts, assets, or properties acquired after your initial funding (special documents such as deeds billed at our normal rate).
• Annual review of assets to ensure both Plan A and Plan B assets are properly under control during life and after death.
• Communication, at your request, with your CPA, financial advisor, or family members.

What is not included:

• Estate plans for non-clients or those not offered this benefit.
• More than four (4) meetings (additional meetings billed at $500/hour).
• Upgrades or additions requiring new circumstances (death of spouse, marriage, divorce, new trusts, business entities).
• Costs such as FedEx, express mail, filing/recording fees, photocopies, and postage.
• Attorney time unrelated to your estate plan.

Enrollment fees are as follows:

• $850 per person if paid by Friday, October 24, 2025
• $950 per person if paid between October 25 and November 21, 2025
• After November 21, 2025, the program will no longer be available for the 2026 year.

All appointments must be scheduled during the 2026 calendar year.
Because of the reduced fee and level of work provided, the fee is earned upon receipt.
